Q. Can patients provide their feedback from a mobile device?
A. Yes, the 2-step patient system has been optimised for mobile devices and desktop computers to make it as easy to access and complete as possible.

Q. How do patients access the feedback system?
A. Patients will receive a postcard along with their samples. This contains a QR code and a simple URL which link to the patient connect system. Illustrated instructions of how to provide feedback are also included.

Q. What do I do if my patient can’t access the internet?
A. We would recommend that you advise your patient to ask a carer or family member to assist them. If this support is not available to them, we can complete the form on their behalf via our helpline.

Q. Which products will my patient be asked to choose from / confirm?
A. The only product(s) your patient will have feedback options for will be the product(s) they have been sent.

Q. Will a prescription automatically be requested once a patient has confirmed their preferences.
A. No, you will be sent their feedback so to that you know which products / flavour they prefer. You will then need to request a prescription if required.

Q. What happens after a patient submits their flavour preferences or confirms that they are happy with a product?
A. You will be sent an email, automatically generated by our secure system, referencing the patients name and their selected products.

Q. Will my patient be receiving any of my details?
A. No, we will not give out your details.

Q. If I send a digital Patient Information Leaflet to my patient, will their email address be retained?
A. No, we do not retain patient email addresses.

Q. How can I preview what a patient will see?
A. This video clearly shows the 2 simple patient steps.

Q. How many samples will my patient receive?
A. The following amounts are typically sent, if you require alternative volumes, please note these in the “Additional requests” box.

Patients will receive the following number of products to sample unless otherwise requested. If you would like your patient to receive more or less, please state this in the ‘additional requests’ box.

  • ProSource Plus / Liquid: 6x 30ml sachets – 2 of each flavour
  • ProSource Jelly: 4 x 118ml cups – 1 of each flavour
  • ProSource 20: 4 x 60ml cups
  • ProSource TF, TF Plant, TF ENFit: 5 x 45ml sachets (TF / TF Plant) or 60ml pouches (TF ENFit)
  • HyFIBER: 7 x 30ml sachets
